Appeal to Witnesses for a Fatal Fail to Remain

Region of Peel – Investigators from the Major Collision bureau are seeking the public’s assistance with the investigation of a Fatal Fail to Remain Collision.

On Friday June 29th, 2018 at approximately 1:00 am, 60-year-old Eddie Chavez Alquinto from Brampton was driving his Mazda 5 vehicle eastbound on Steeles Avenue in the area of McLaughlin Road.  His wife 45-years-old was a passenger in the vehicle.  As they proceeded eastbound their vehicle was violently struck from behind by a white Mercedes Benz, driven by 30 year old Martin Hines from Orangeville. 

The impact resulted in the immediate death of Alquinto; his wife sustained serious injuries and was transported to a Toronto trauma centre. 

Hines exited his vehicle and fled the scene in a black vehicle taken from the scene.  He was eventually involved in two additional collisions in the Toronto area; one of which resulted in life threatening injuries to a 20-year-old male.  Hines was arrested by Toronto Police Service and is being held for a bail hearing.

Hines is facing multiple charges related to the collision including Dangerous Driving Causing Death, Dangerous Driving Bodily Harm, Fail to Remain Causing Death, Fail to Remain Causing Bodily Harm, Theft and Robbery.

Investigators are appealing to any witness who may have observed the accused’s driving prior to the collision or have dashboard video footage leading up to the incident.
