To obtain a copy of a Police Occurrence Report, please mail in the following:
- Completed Form D
- Copy of 2 pieces of Government issued photo identification (front and back of identification)
- Payment of $33 (payable to Peel Regional Police, via certified cheque or money order). Note, the fee will be $33.00 beginning January 1, 2023.

Requests will not be processed without all of the above.

As of May 1, 2023, the option to ‘pick up’ Form D requests for police reports will be discontinued. The options to receive your reports will be mail or email only.

Citizens can visit in person at Peel Regional Police Records Services Reception Desk, to obtain a copy of a police report. Bring a Form D, two valid pieces of government issued identification and payment as indicated below.

Form D

If you are unable to attend in person, you are required to mail us a request, along with notarized copies of two pieces of government issued identification (i.e. passport, driver's license, etc.) and a money order or certified cheque made payable to the Region of Peel. Details of the incident and/or police report number should be included as well.

The report will remain available for three months from the date it was completed. Any police report not picked up by that date will be destroyed. The Peel Regional Police Service will not reprocess these reports or refund any fees. We accept cash, Debit Cards, MasterCard and VISA.

Motor Vehicle Collision

  • You must have been involved in the collision in order to obtain a copy of the Motor Vehicle Collision Report.
  • Service fee of $40.

How to Request Records Contained in a Major Collision File 

Major Collision Bureau investigates all Motor Vehicle Collisions resulting in a fatality or serious personal injury that occur in the Region of Peel. 

By Mail or Email

  1. Company letter or personal/company email requesting the records, including date, time, location, occurrence number, officer in charge of the collision.   How you are involved or your relationship to the involved party.
  2. Client’s signed consent or two copies of your government issued identification and your contact information.
  3. Our office will advise you in writing regarding the status of the case or any known court dates.

If you are NOT the Involved Party to the Collision

You must provide proof of relationship to the involved party and a signed consent on behalf of the involved individual. 

If the Involved Individual is Deceased

You are required to provide an authorization from the legal representative or power of attorney with supporting documents.

Occurrence Synopsis Reports

  • e.g. Break and enter, stolen vehicle, theft from a vehicle, lost/stolen property (including identification and passports), mischief, fire, theft, assault, domestic, fraud/identity theft, landlord/tenant dispute, assist citizen, missing person, robbery, neighbour dispute and dog bite.
  • Service fee of $32. Note, the fee will be $33.00 beginning January 1, 2023.

  • Monday & Tuesday – 8 a.m. to 3:30 p.m.
  • Wednesday & Thursday – 8 a.m. to 6 p.m.
  • Friday – 8 a.m. to 3:30 p.m.
