Passport Reports

 

Individuals attend in person to Records Services Reception Desk to request a copy of a Police Report regarding lost or stolen foreign passports, or stolen Canadian passports in which they are involved. ** Please note that an individual must first report the lost/stolen passport to an officer at any division before they can request a copy at Police Headquarters.**

If you have LOST your Canadian Passport in Canada, you must report it to Citizenship and Immigration Canada's Passport Program. The report needs to be submitted by phone at 1-800-567-6868, or in person at the nearest Service Canada Passport Office.

If your Canadian passport was STOLEN you must report the theft to police and to Citizenship and Immigration Canada's Passport Program.

A Peel Regional Police Records Services request Form "D" is required. The form is available in the lobby of Headquarters or print your own copy. These reports take approximately ten (10) to fifteen (15) days to process.

Applicants will be required to produce two pieces of valid, government issued identification. At least one piece must include a photograph (i.e. passport, driver's licence, etc).

SERVICE FEE $45.00 (effective January 1, 2017)

We currently accept cash, Debit Cards, Mastercard and VISA.

Business Hours to make a request or pickup a completed request:

Monday through Friday 
8:00 a.m. to 6:00 p.m.

Saturday 
9:30 a.m. to 4:30 p.m.

Closed Sunday and Statutory Holidays

 

The report prepared as a result of your request will remain available for 3 months from the date it was completed. Any police report not picked up by that date will be destroyed. The Peel Regional Police Service will not reprocess these reports or refund any fees.
